7:00AM – 8:00 AM

Welcome Address

Join us as SASM President Dr. Bhargavi Gali opens the Annual Meeting with a warm welcome, setting the stage for a day of learning, collaboration, and innovation in perioperative sleep medicine.

8:00AM – 8:10AM

Pediatric POCUS

Point of Care Ultrasound is a tool that has become ubiquitous in adult ICU, the introduction of smaller probes and the ability to diagnose and manage pediatric patients rapidly is gaining interest and success. This lecture will deal with a variety of POCUS examinations that can be easily used and implemented in everyday practice.

About Dr. Santhanam Suresh

Dr. Suresh is a Pediatric Anesthesiologist and Pain Medicine specialist who specializes in acute pain management and regional anesthesia in children. He is vastly published in many areas in pediatric anesthesia and has served in leadership positions in the American Board of Anesthesiology, International Anesthesia Research Society, American Society of Regional Anesthesia and PAin Medicine, and Society for Pediatric Anesthesia.

Dr. Santhanam Suresh

8:15AM – 8:40AM

Pediatric OSA Panel

Pediatric obstructive sleep apnea presents in up to 7% of children and represents a constellation from nasal turbulence to cessation in gas exchange. There are numerous end organ sequelae including neurocognitive morbidity associated with persistent OSA.1 In young children, adenotonsillar hypertrophy is the most common anatomical abnormality associated with OSA, and adenotonsillectomy is, therefore, the most common surgical intervention. Perioperative complications associated with adenotonsillectomy are more common in children with severe OSA. The safe anaesthetic management of a child with OSA requires an anaesthetic technique tailored to the underlying aetiology and severity of OSA and the surgical procedure.

Dr. Ali I. Kandil

Dr. Nicholas Rahn

Dr. Kirti Sahu

8:45AM – 9:25AM

About Dr. Ali Kandil

I finished my training at Robert Wood Johnson/Barnabas Health and completed pediatric anesthesia fellowship at Cincinnati Children’s Hospital Medical Center. I’ve been faculty at Cincinnati Children’s Hospital Medical Center for over 10 years, where I have dedicated the majority of my time to anesthetic management of patients undergoing diagnostic procedures in radiology for the evaluation of obstructive sleep apnea. 10 years ago, our team published a protocol for drug induced sleep endoscopy (DISE) and Cine MRIs of the upper airway that remains the standard for these procedures in terms of anesthetic management. We have published additional papers regarding nuance of how and where these procedures are performed. Additionally, I have interests in intraoperative neurophysiology and quality improvement and have given lectures and published papers in these fields.

About Dr. Nicholas Rahn

Dr. Rahn is an Assistant Professor of Anesthesiology at Cincinnati Children’s Hospital, where he recently completed fellowship in Pediatric Anesthesiology. Dr. Rahn completed his residency training at the University of Florida in Gainesville.

About Dr. Kirti Sahu

I am assistant professor at department of anesthesiology at CCHMC. My area of interest are airway management, anesthesia for fetal surgeries and POCUS.

9:30AM – 9:45AM

Monitoring Unconsciousness and Brain Health During Anesthesia

How do anesthetic drugs produce unconsciousness? Can we actually measure anesthesia-induced unconsciousness with the EEG? And what might the EEG tell us about the underlying “brain health” of our patients? Brain function requires an orchestration of activity across different levels or organization, from neurons to circuits and systems, that is mediated by brain rhythms or oscillations. Anesthetic drugs disrupt the brain oscillations involved in sensory processing, cognition, and arousal, and we can see these effects in the EEG. In this talk we will discuss the relationships among brain oscillations, brain function, anesthesia, and brain health, and give participants practical tools they can use in the operating room to improve patient care.

Empty Stomach No More: GLP1 Agonists and Perioperative Management Panel

GLP-1 receptor agonists, increasingly used for diabetes, obesity, and related conditions, can delay gastric emptying and increase the risk of aspiration during anesthesia. Standard fasting guidelines may not fully address these risks. While evidence-based guidelines are limited, consensus suggests preoperative withholding of GLP-1RAs is generally not recommended. Strategies to enhance safety include a liquid diet for high-risk patients, low threshold for gastric ultrasound, and consideration of rapid sequence induction when appropriate. Perioperative management should involve shared decision-making between anesthesiologists, proceduralists, and patients. This session will review current data on gastric ultrasound, its practical use, limitations, and application for aspiration risk assessment in patients on GLP-1 therapies.

Reaching Underserved and Under Resourced Populations for OSA

OSA and Insomnia remain vastly underdiagnosed in underserved and under-resourced populations despite its high prevalence and significant health impact. The traditional sleep care model has gaps and barriers which make it difficult for those with socio-contextual realities such as economic challenges, unsafe neighborhoods, caregiving responsibilities, low educational attainment, medical debt, and shift work. There is also a lack of a wider focus on those with physical and intellectual disabilities regarding sleep health and sleep disorder treatment. Also, systemic issues like prohibitive testing costs, fragmented referral pathways, and lack of culturally relevant care keep patients from accessing OSA and Insomnia treatment. The field of sleep medicine and sleep research continues to innovate as new treatments emerge for sleep apnea. That innovation spirit can be extended to other health professionals who work in communities across the United States. Community health workers and social workers are uniquely positioned to bridge these gaps by delivering tailored sleep health education and linking individuals to appropriate care. Trauma-informed approaches are also a gap in standard care. This discussion will touch on these types of approaches and suggest a number of other care system collaborations which may help address the gaps for those who are underserved and under resourced. The shared goal of improving health outcomes and addressing health disparities is a commonality across the healthcare system. Reaching underserved and under-resourced populations will require clear strategies, cost effective solutions and training other health professionals.

David is a sleep patient advocate and the founder and CEO of the Sleep Equity Project. His mission is to make healthy sleep accessible to everyone, regardless of where they live, what resources they have, or their gender or age. His work focuses on raising awareness, providing education, and developing programs to address sleep health disparities, primarily in Chicago. Some of the programs he has created include:
• A sleep education training program for community health workers
• A sleep disorder screening and referral program for returning citizens after incarceration in prison
• Collaborating with a safety net hospital to build sleep disorder treatment resources
• Teaching middle school students in Chicago about healthy sleep habits
He also participates as a stakeholder on multiple PCORI grants and also has participated as a PCORI reviewer. He currently serves on the AASM Foundation Community Award Committee as the Chair. In July 2024, David was appointed to the NHLBI’s Sleep Disorder Research Advisory Board for a 4-year appointment. David was fortunate to serve on the most recent AASM Hypopnea Scoring Rule Taskforce as the only patient representative in an effort to address the issue of low awareness among patients. In addition to having sleep apnea himself, he has multiple family members who have also been diagnosed and is passionate about the generational impact of sleep disorders on relationships and the quality of life. He also is interested in OSA issues around the lack of awareness around the impact of hypoxemia and gender differences in clinical presentations.
